What is acceptance and commitment therapy?

ACT (said as one word, like the verb) is a form of talk therapy built on a simple idea: trying hard to avoid painful thoughts, memories and feelings often keeps people stuck. The Association for Contextual Behavioral Science describes acceptance and commitment therapy as an approach that uses acceptance and mindfulness strategies, together with commitment and behavior change strategies, to increase psychological flexibility. In plain words, psychological flexibility means being able to notice what is happening inside you, make room for it, and still choose actions that fit the person you want to be.

What does an ACT session look like?

ACT sessions are conversations, with a few more exercises than you might expect. Your counselor may use metaphors and short experiential exercises to help you practice skills such as:

  • Stepping back from thoughts: noticing a thought like "I always mess this up" as a thought, rather than a fact you have to obey.
  • Making room for feelings: letting anxiety, sadness or grief be present without letting it make every decision.
  • Clarifying values: naming what you care about in relationships, work, health and community.
  • Committed action: choosing one small, concrete step toward those values and checking in on how it went next session.

Some Foundations counselors blend ACT with other approaches, such as cognitive behavioral therapy (CBT) or mindfulness practices, depending on what fits you.

What can ACT help with?

People often come to ACT when they feel stuck: worry that will not quiet down, low mood, perfectionism, avoidance, grief, or a life that has shrunk around a problem. Counselors who use ACT see it alongside anxiety counseling and depression counseling. The research picture is still developing. A Canadian health technology review of ACT for PTSD, anxiety and depression found that results were somewhat conflicting depending on the population and study design, and that in survivors of interpersonal trauma, ACT reduced symptoms of PTSD and depression. Your counselor can talk with you about whether ACT, another approach, or a mix makes sense for what you are facing.

How is ACT different from CBT?

Both are practical and focused on the present. Traditional CBT often works by examining a distressing thought and testing whether it is accurate. ACT spends less time arguing with the thought and more time changing your relationship to it, so the thought has less pull on what you do next. Neither is right for everyone, and many people find parts of both useful.

What does ACT counseling at Foundations not cover?

Foundations offers one on one outpatient talk therapy. It is not crisis care, and it is not a replacement for a physician or psychiatrist. Answers to Common Questions

Does acceptance mean giving up or putting up with a bad situation?

No. In ACT, acceptance means making room for thoughts and feelings you cannot switch off, so they stop running the show. It does not mean approving of mistreatment or staying in a situation that hurts you. Often, people find that once they stop spending all their energy fighting their feelings, they have more energy to change the things they can change.

Will I have practice to do between ACT sessions?

Usually a little. Your counselor may suggest a short mindfulness exercise, noticing when a familiar thought shows up, or one small action that fits your values. These are suggestions, not assignments you can fail. You and your counselor decide together how much practice fits your week, and you talk about what happened at the next session.

How long does ACT counseling usually take?

It depends on what you are working on and how often you meet. Some people come in for a focused stretch of sessions around one problem, while others use ACT as part of longer counseling. Your counselor will check in with you about progress toward your goals and talk openly about when it makes sense to space sessions out or wrap up.

What are the costs associated with counseling or therapy sessions at Foundations?

At Foundations, each counseling or therapy session is $200. Your first consultation is always free, and you are able to meet with as many counselors as you need until you find the perfect match.

Does Foundations accept Insurance?

No. Foundations is private pay and does not bill insurance companies. If your plan has out of network benefits, we can give you documentation to submit for possible reimbursement, so check with your plan first. Paying privately keeps your counseling out of insurance records, avoids a required diagnosis, and lets you choose your counselor and how long your sessions last.

What if I need to reschedule my counseling session?

There is no charge for any session cancelled at least 48 hours prior to the scheduled appointment time.

Are you available in the evenings or on weekends?

Yes. Foundations offers early morning, daytime, evening and Saturday appointments, and online sessions can add flexibility. Call 970.227.2770 or book online, tell us what times work for you, and we will match you with a counselor whose schedule fits.

Can ACT help me cope with a health condition or chronic pain?

ACT is often used to help people live well with things that cannot simply be fixed, including the stress of pain or a long term health condition. Counseling can support the emotional side, such as frustration, fear and grief over what has changed. It works alongside your medical care, not instead of it, so keep your doctor involved in decisions about treatment.

Do I need to know anything about ACT before I start?

No. You do not need to read about ACT or practice mindfulness beforehand. Your counselor will explain each idea as it comes up, in everyday language, and connect it to your own life. If an exercise feels awkward or does not fit you, say so. Good ACT work is flexible and adjusts to the person in the room.

Can ACT help if CBT did not work for me?

It might. CBT often focuses on changing unhelpful thoughts, while ACT focuses on changing your relationship with them, so you can notice a thought and still act on your values. Some people who felt stuck arguing with their thoughts in CBT find this shift useful. Your counselor will ask what you tried before and what did and did not help, then shape the approach around you.
