What does the job do to people over time?

Most responders handle single hard calls well. The weight usually comes from accumulation: years of scenes you cannot unsee, sleep wrecked by rotating shifts, and the habit of staying calm while everything inside runs hot. A SAMHSA research bulletin estimates that about 30 percent of first responders develop behavioral health conditions such as depression and PTSD, compared with 20 percent of the general population. Common signs are trouble sleeping, irritability, replaying calls, drinking more to come down, and feeling numb or far away from the people you love. The National Institute of Mental Health suggests getting professional help when distressing symptoms last two weeks or more.

What is moral injury, and is it the same as PTSD?

Not every hard call leaves fear behind. Sometimes it leaves guilt, shame or anger: a call where policy tied your hands, a decision you keep second guessing, or watching something happen that went against everything you believe. The VA National Center for PTSD describes moral injury as what can happen after someone takes part in, fails to prevent or witnesses an event that goes against their morals or values, and notes that first responders can experience it. It can overlap with PTSD symptoms, but it often needs a different conversation, one about values, responsibility and forgiveness rather than only about fear.

How does the job follow people home?

Hypervigilance that keeps you safe on duty can make a restaurant or a kid's birthday party feel exhausting. Partners often say they get the leftover version of you, short tempered or shut down after a long run of shifts. Counseling can help you build a real off switch, talk with your partner about the job without bringing every scene home, and notice when anger or drinking has started doing the coping for you. Some couples also choose couples counseling together.

What does counseling look like for a first responder?

Sessions are direct and practical. You set the pace, and you do not have to retell every call in detail to make progress. Counselors who work with first responders often use Cognitive Behavioral Therapy (CBT) to work with the thoughts that keep a call replaying, along with trauma focused approaches, sleep and body calming skills, and plain conversation about identity, purpose and what you want your career to look like from here.

Will my department find out I am in counseling?

Foundations is an independent private practice, not part of any agency, and you reach out to us directly. What you share stays between you and your counselor, with the legal exceptions every counselor must follow, such as an immediate risk of harm. Your counselor will walk through those limits in your first session. If your agency requires documentation for something, that only happens with your written consent.

Do I have to talk about the worst calls?

No. You choose what to share and when. Many responders start with what is happening now, such as sleep, irritability or tension at home, and only talk about specific calls once trust is built. Progress does not depend on retelling every detail. Your counselor will check in about pace so sessions feel steady rather than overwhelming.

Can counseling help with shift work and sleep?

Yes. Rotating and overnight shifts disrupt sleep for almost everyone, and poor sleep makes stress, mood and patience worse. Counselors can help you build wind down routines, handle the racing mind after a late call, and plan around your schedule. If sleep problems persist, see your physician too, because some sleep issues need a medical evaluation.

What are the costs associated with counseling or therapy sessions at Foundations?

At Foundations, each counseling or therapy session is $200. Your first consultation is always free, and you are able to meet with as many counselors as you need until you find the perfect match.

Does Foundations accept Insurance?

No. Foundations is private pay and does not bill insurance companies. If your plan has out of network benefits, we can give you documentation to submit for possible reimbursement, so check with your plan first. Paying privately keeps your counseling out of insurance records, avoids a required diagnosis, and lets you choose your counselor and how long your sessions last.

What if I need to reschedule my counseling session?

There is no charge for any session cancelled at least 48 hours prior to the scheduled appointment time.

Are you available in the evenings or on weekends?

Is counseling available for spouses and families of first responders?

Yes. Partners and families carry the job too: missed holidays, worry during shifts, and a loved one who comes home changed by a bad run. Foundations offers individual counseling for spouses, couples counseling for both of you, and family counseling when kids are feeling the strain. Tell us who is coming in when you reach out so we can match you well.

Can I see a counselor around my shift schedule?

Often, yes. Online sessions anywhere in Colorado make it easier to fit counseling between shifts, and many counselors offer a range of appointment times. Share your rotation when you reach out, and we will look for a counselor whose availability lines up with yours, so you are not forced to choose between sleep and a session.

Can I come in after one bad call, or only when things are bad?

You can come in any time, including after a single call that will not leave you alone. You do not need a diagnosis or months of symptoms first. Talking early can help you sleep, process what happened and notice warning signs before they build. Many responders also use counseling as regular maintenance, the way they would train or service equipment.
